A death certificate is a document issued by the Government to the nearest relatives of the deceased, stating the date, location, and cause of death. This certificate mainly serves as proof for legal purposes to collect medical benefits, pension benefits, claiming life insurance, and other official affidavits. Under the provision of the Registration of Death Act, 1961, it is important to register the death certificate under the State government of Uttar Pradesh. It is an important legal document that is issued by the Urban department to certify the demise state of any individual or person.

Documents Required:
- Application form
- Photocopy of any one of the passport / ration cards / self-certified declaration/voter card / PAN card / driving license / nationalized bank for identity
- Passport size photo of deceased
- Proof of death of a person
- An affidavit, of death, is being registered after one year.
How to apply for a death certificate online in UP: Online
- Visit the official website of the Uttar Pradesh Government http://e-nagarsewaup.gov.in/.
- Now under “Citizen Services” click on the “Citizen’s Login” option.
- Then click on “New Registration”.
- Once the registration is completed, log in again into the portal and click on the link death certificate to apply.
- Now fill in all the details in the application and upload the scanned documents.
Note: The applicant will receive an SMS to the registered mobile number as an acknowledgment.
How to apply for a death certificate online in UP: Offline
- Visit the nearest CSC center.
- Obtain an application for a Death Certificate.
- Fill in the details mentioned in the certificate, accordingly.
- Attach the specified documents
- Submit the application form to the CSC operator.
- You will receive an SMS on your registered mobile number.
- Finally, make payment to process your application to the Nagar Nigam, and it is done.
